Please submit the interest questionnaire on the contact page, briefly detailing the services that your district is interested in.
Step #2:
INITIAL MEETING - 60 minutes
Once an interest questionnaire is completed, Canella will review and follow up via phone or email to schedule an initial meeting. During the initial meeting, Canella can learn more about the services that your district is seeking. This is also a time to ask questions regarding services, fee schedules, and any specifics regarding the drafting of the contractual agreement.
Step #3:
CONTRACTUAL AGREEMENT
A contractual agreement is drafted and sent for review. Once services and terms are agreed on the contractual agree is signed by both parties.
Step #4:
CASE CONSULTATION
Relevant school personnel and IEP team members are contacted to gather the necessary information needed to inform the evaluation. Testing dates are scheduled.
Step #5:
REPORT WRITING
Once all of the necessary evaluation pieces are collected (e.g., testing, rating scales, interviews, developmental history, neuropsych report), a write-up is generated and sent to the case manager in PDF format within 12 schools days after testing.
Step #6:
MEETING ATTENDANCE
Should your district prefer my attendance at the student's evaluation meeting to review the evaluation document, a meeting date/time is agreed upon with school staff and parents to ensure availability.
